This patient has Dyspnoea, Orthopnoea and odema. What are your thoughts on seeing this CXR?

This is a globular heart indicating a pericardial effusion.

The X-ray shows globular cardiomyopathy might indicate pericardial effusion, dilated cardiomyopathy or multiple valvular disease.
